example visa lettre complete documents

Dear xxx

I sent all the required documents from my site but now you have to complete the some documents which are:

1. One visa application form completely typed (typewriter/computer) with complementary page. (First printout n give Anita to fill then type n print on at the same time.)
2. Two recent passport-sized photographs with light background. Dull or blur photographs are unacceptable. (it should be more natural no make up n no big tops/earrings) 
3. Passport valid for at least three months beyond the intended stay and previous passport/s. 
4. One photocopy of passport pages with information & Schengen/UK/US visa.(front page n where hinduism written) 
5. Self-declaration by the applicant on true statement.(please can again confirm from them what does it means) 
6. Proof of an existing round trip air ticket (reservation / booking slip etc) (please can u make reservation for 16april 2010 n returns in 15 July 2010 we will buy ticket when we get visa)
7. Proof of existing travel-health insurance valid for the Schengen states and covering a minimum amount of 30,000 Euros for medical treatment and home repatriation in case of illness or death.  (please do with your friend who is working in efu n clearly mention what is required for our visa)

Please read above all brakets() the things please follow them and complete all the documents.

You can do it next week when my documents reached you.
